APPOINTMENTS

Step 1 :: Appointment Scheduling
You may fill out an Appointment Request form electronically and someone from our office will call you to schedule your appointment, or we can email you with an appointment. Our office is open Monday through Thursday from 7:30am to 4:30pm., Friday 7:30am - 12:00pm with referral by your Pediatrician being recommended. An appointment for a routine evaluation can usually be obtained quickly, often within the week. For urgent appointments please call the office and press "1" to speak to someone directly.

Step 2 :: Patient Registration
Prior to your appointment, please download or submit online a Patient Registration form and a Medical History form.

Step 3 :: Preparation / What To Bring

  • Please bring any authorizations or prescriptions. Your appointment may be rescheduled if you do not have proper authorization to be seen.
  • Please bring your current insurance card and photo ID.
  • Review other suplemental informational forms provided for you.
  • Review information about Your Childs First Visit?
  • Cancellations
    Please call us at least 24 hours in advance if you need to reschedule or cancel your appointment; this will allow another child to see us in a timely fashion.


    Hearing Problems
    If you think your child has a hearing related problem or may need a hearing test, please notify the receptionist at the time of your call or online appointment request. A separate appointment with the audiologist will be arranged. Some insurance companies consider the audiologist as a separate care provider and may require us to charge a second co-pay.

    X-Rays
    If you think your child needs X-Rays for the specialist to properly evaluate your child, please let the receptionist know at the time of your call. We may be able to obtain them prior to your appointment.
